whats wrong with RNG?
Every ######### time hatch must spawn near crawling survivor. Same with hex totems. They ALWAYS spawn near gens or loops.
Is there some RNG pattern or what?

Nope. The clue is in the name. I'm not sure what you mean about the hatch. Perhaps you've just been unlucky, but in my experience as a crawling survivor, and just mathematically speaking, the chances of it spawning right next to a downed survivor are actually quite low. As for the totems, aside from being just completely out in the open (which I'm sure is the last place you'd want your totem to be), you'd be hard pressed to find a single spot on any map that isn't close to a generator or a loop.

If game or more like maps were created randomly there would be a total mess. Its not a seed generator like in minecraft. In this game, devs have choosen the spots.
Every chest, mountain and hook spawn randomly but in spot that was made before by devs themselves.

I... know that. Or at least, I understand that the map generation works the way Peanits said. There are still a good several dozen or more spawning spots for both totems and hatches. I'm not sure what your point is.

If spots are choosen manually, why in the name of holy crap are totems placed next to generators?

Because the devs didnt think about what if a gen spawned at that totem location
