A few days ago i noticed that this perk is bugged, when i'm injured the perk appears as activated but it is not really working i could hear my survivor cry in pain.

Is Stridor supposed to make the grunts louder from the survivor perspective as well?

Yes. Both grunts and breathing is louder for everyone including yourself.
