Item add-ons should NOT be lost IF a survivor picks up a new item in-game with add-ons.
Assumptions:
Survivor must escape with the new item.

Ace in the Hole allready does that.
Other than that (picking item from another Survivor who bring add-ons with it) I don't think this should be a thing.

I think it should be based on charges used.
25% charges used : Higher rarity add on lost
75% charges used : lower rarity add on lost

Killer and survivors are equal now keep it that way

with some little difference:
- the "base kit" of survivor (toolboxes, maps ecc) will gones if survivor dies.
- major of the item's add-on are trash.
-in the bloodweb can find more garbage or base-kit that utility add-on.
take for example a flashlight: all add-on except battery are garbage.
maps and add-on are "fine" for newbie.
toolboxes and their add-on are useless (they deserve to be nerf, but now are only more useless things on bloodwebs).
85% of the survivor bloodwebs now are completly useless.
