Why DbD will never be “fixed”
Pick out anything in the game. It has a problem with it and I do quite mean literally everything, so to avoid a wall of text I’m just gonna put down the things that don’t seem to have a problem.
Skins: most are too expensive for casual players and also too grindy for the very few that come out that are worth grinding for
Map visuals: There are still plenty of maps in the game in need of serious visual overhauls, the most grevious offender being mount ormond with cloudy visuals, tanking frame rate, washed out auras, hard to see scratch marks, and an overall dull color palette.
Music: In the mission to make all killers have their own themes, at the pace of 1 killer per 3 months... we are looking at 14 killers if my math is right. Meaning 42 months or 3 and a half years. Which is almost the lifetime of the actual game so far. So it is unlikely all the killers will ever get their own themes.
Everything else you could name I feel I could super easily find an issue with it. And with all the issues in the game there is no physical way a team the size of BHVR could fix everything.
This is why 2 straight years with nothing but band aids hurts in the long run.

Thing is, theyre just cosmetics.
Listen, as far as Bandaid fixes go, its kept the game together so far. Some of those fixes have been replaced by actual changes.
As far as chase themes go? I would rather be allowed to edit my music file for DBD to use the chase music I want.

Tbh the band aid fixes did take time away from the games lifespan because instead of making real changes to the game the first 2 years they gave us perks and badly designed killers to compensate for the poor design flaws of this game. They're making changes now which is a step in the right direction but as you have said, new chapters and content will get in the way of making the game's balance better.

The thing about that is that you can just edit in complete silence so you have absolute hearing of everything around you, making it much harder for survivors to stealth you.

if only the content release cycle wasn’t 3 months
you shouldn’t have to wait months for simple perk or balance tweaks, it’s why the meta has gotten so stale

Didn't the same thing happened with PAYDAY 2 but on a more large scale?
I remember people always bringing the Thanatos Sniper Rifle and Brothers Grimm Akimbo Shotguns because they are objectively the best guns in the game and changing away from it makes Death Sentence impossible

As far as skins, I was hoping that the cosmetic sale they had in January might recur more often (where they sold several cosmetics for 300, 600, and 900 auric cells). It hasn’t come back yet but I still have my fingers crossed.

Payday 2 has more viable off meta stuff
you can go max armor and LMG and get by or go full shotgun dodge Crit and never go down
DBD doesn’t have as much flexibility as many perks are flat out objectively useless or require a great deal of effort to activate

That is true, there are some perks you no longer need if you've played the game enough like Deja Vu.

And that is why it will continue to do so. While I get many people will complain about anything for any reason, its the complacency of others that lead (for lack of a universal name) developer of goods to no longer feel like quality matters.
I mean why actually spend time and resources to fix something if people will just accept our broken mess?
