Skill Check/ Sound Glitch

So I've played a few games recently where I'll be doing gens/healing and I'll get the skill check sound but nothing pops up. Then I either blow up the gen/stab myself or the person I'm healing... Has this happened to anyone else lately or is it my internet?? I also notice that the sounds are weird, like someone will run over to heal me and I still hear them breathing heavy like they're running...
I just wanna know if anyone else has this problem.
