Invisible Torment Trails
Has anyone else had this problem? I'll be walking and get tormented even though there isn't a trail. I don't believe there is an addon for it.

Yeah had this issue now several times. Sitting on a gen and getting randomly tormented or walking around with nothing on the ground, not even Grass, and getting tormented suddenly.

Yes, few times the floor was clear - no grass or other objects to obscure any trails and boom...hit with torment. I was once healing someone else near a trail, but clear of it...halfway through the heal, boom hit with torment for no reason.
Since they made it downright vital to avoid torment when facing Pyramid Head, they should ensure that the decay of the trail is total once it vanishes...or make it not affect when on the final stage of decay.

Only dealt with it once and that was on the new map.

Yeah I've had it happen a few times too. It's like there is the tiniest spot of PH's trail, but it's hidden below the map or something. It's really frustrating when it happens.

They have alot of bugs with the Torment trail. I was being healed by 2 other survivors and halfway through I randomly became Tormented when there was no trail in site and Pyramid Head was chasing someone in opposite side. I was upstairs on the game map when he was chasing someone below me - barbed wire popped up on the floor upstairs in front of me when he put it down on the floor below. He put a trail going into the killer shack, I was working a gen and the trail dissappeared but the sound it made continued.

It does happen and can be annoying.

It's been there since the PTB. You can be on a gen, and you suddenly get Tormented.
They haven't even acknowledged it.

It's still a problem. It happened a few days ago, I was cleansing a Dull Totem and everything was fine and right after I was randomly afflicted by Torment. Just to clarify, nothing was on the floor.

Yeah it’s very similar to some bugs that plague had in the past of random objects and random survivors just suddenly becoming infected.
