Dead Hard only after being injured?

As you can see, I'm not exhausted or anything. Perfectly healthy and running. The prompt for DH doesn't show until after I'm injured. This is how it always is for me. Is this normal?

Also, look at that lag.

  That's intended, the option wont show up until you're injured.

  The description even specifies that you must be injured

  Thanks, guys. I absolutely didn't even realize that the description says "When injured," for some reason my eyes just skipped over that.
