Surge is the best regression perk with Ruin
Ruin is a risky perk that can be extremely good, but you always have to run it with a backup perk for gen control.
Corrupt gives the Survivors time to look for Ruin and Pop doesn't work until Ruin is down. BUT Surge activates on any generators that aren't regressing so it can explode nearby generators if other Survivors are working on them.
So Surge is pretty much the only gen regression perk that has strong synergy with Ruin because it can get value if Ruin is gone or if it's still active.

The only problem with Surge is that it has a unlogical cooldown when it doesn't even trigger regression. If it triggered regression I would be fine with cooldown.

The Surge cooldown is fine. Usually it doesn't come into play. I just wish Surge had the Infectious Fright treatment and activated whenever Survivors were put into the dying state instead of only with basic M1 downs

I'd say sloppy butcher is actually the best regression perk with ruin.
8% regression to the gen is....6.4 seconds in theory? And that's with a 40 second cooldown and also requires you to down someone near a gen someone else is working on one nearby. That's alot of conditions to be met when sloppy has no cooldown and is an extra ~4 seconds added to healing on any basic attack. It also encourages people to stay off gens in the first place, letting your ruin get more value if it doesn't die in the first 40 seconds of the game (which it will alot of the time).

It seriously doesn't trigger regression? I swear it did when I was using it back before Nurse nerf... strange.
Yeah without regression there is literally no reason to have a CD on this. Even my best games as Nurse with back-to-back downs would still never benefit to the point of being broken without a CD on surge. It might actually make me run it again without a CD.
I've always thought they had bad synergy because I was looking for perks that worked from the start of the game till the end of it but you make great points. Even with Ruin up it's still useful for information on gens (even if it'll be rarer) and it works when Ruin does fall.
You've changed my mind on it OP. Not enough to run Surge again with the CD but if that ever changes I will definitely run that combo.

What I'm saying is that makes no sense to give the perk a cooldown if it doesn't even trigger regression when it activates.

Meh. Surge is better as a standalone perk than as a Ruin enhancer.
If you're going to combo Ruin with something then I'd try Dying Light. It slows gen repair speed and healing speed, meaning it takes Survivors longer to repair gens and longer to heal allowing gens more time to regress as well.
That being said, Surge is decent as a standalone on specific Killers. Specifically Myers, Spirit and Ghostface can get a lot of value out of the perk since they can quickly pop up in front of a Survivor and immediately down them triggering the perk before they can start running in the first place.
An important thing to note is that a lot of the value from the perk is how it saves time. This is why it's not a great ruin combo, because that aspect has overlap with ruin and it doesn't stack. So while you get SOME value out of it unlike PGTW, you still lose the most important part of the perk.
Though to be fair, it's still good even if a Survivor is still on the gen when it's popped, since not only does it regress the gen by about 6 seconds, but it also has a pause effect like with a missed skill check, which stalls the gen for about 2 seconds. This doesn't mean much if it's just one Survivor on a gen, but if there are several Survivors on a gen then that's easily double the effective stall power from that pause, that kind of thing can easily mean the difference between the gen popping and you getting to them and chasing them off.
