PS4 - Tormented but no trench

I was injured and just saved someone off the hook. Another survivor came over to help heal myself and the other injured survivor. They worked on healing me first, when I was about 50% healed all of a sudden I was inflicted with Torment even though there was no trench and Pyramid Head was on other side of map chasing someone. After they finished healing me we healed the other injured survivor in the same spot, she did not become Tormented.

Steps to recreate

1. Play in public match as survivor against Pyramid Head. I was Cheryl Mason, Map was Azarov's Resting Place.

2. Be injured, rescue survivor from hook, be healed by that survivor and another.

    This also happens while repairing a generator. It happened multiple times, but we didn't know what was causing it. I'll check to see if perhaps it is happening when the gen passes 50%.
