Survivor stuck right before getting hooked

The survivor was stuck in place like in the first screenshot. They couldn't be healed, unhooked, picked up, or anything. When spectating them, it actually just spectated the killer. Endgame collapse did not finish the game, the survivor had to disconnect for the game to end.
Also possibly related, but in the same game the nurse did something weird. She blinked once, and then hit me, then immediately blinked a second time with her chain blink.

Blinking immediately with a chain blink is possible by an addon,it's called "Campbell's last breath" ^^
