Where can I read the patch notes?
I want to figure out how fast I will be healed if I have leader and two people are healing me.
But I know co-op actions have a slight debuff of some sort? Something to do with the charges per second?

16-25%= 12 Normal heal with leader ( 1 person)
8 - 50%= 4 seconds( 2 people), my math may be wrong
And you will only receive a debuff when stacking on generators.

Nah How it works (if no co-op debuff to heals)
16 sec ÷ 1c/s = 16 second
16 sec + 25% bonus would be...
16sec ÷ 1.25c/s = 12.8sec
So two survivors would be...
16 sec ÷ 2.5c/s = 6.4sec

You won't find that in the patch notes, but if you're looking for patch notes, you can either go to the patch notes section of the forums (from the front page or on the right), by clicking the Patch Notes link in the side panel under Useful Links, or by heading to the Info & Help button at the top of the page.
To answer the healing question though:
- Each survivor adds 1 charge per second (CPS) by default, and the heal takes 16 total.
- Leader adds 0.25 charges to each of them, so each survivor adds 1.25 CPS.
- 1.25 CPS x 2 survivors = 2.5 CPS
- 16 / 2.5 = 6.4 seconds to heal
Assuming there's no Sloppy Butcher or anything slowing it down, of course. There is no inefficiency to co-op actions on anything but generators.

Question- If I'm affected by 2 different Leader perks (ie person I'm healing and person helping me heal), is it capped at 25% (1.25c/s), does it increase to 50% (1.5c/s), or does it increase the 1.25 by another 25% leading to 56.25% (1.5625c/s)?

In Leader's case, you can only be under the effect of one Leader perk at a time (with the higher value taking priority). Though as a general rule, positive modifiers (e.g. increased speed) stack additively, while negative multipliers (e.g. decreased speed) are multiplicative. If you were able to stack two Leaders, for example, each survivor would be healing at a rate of 1.5 CPS.
