It's weird because he has a lot of negative synergy with so many perks. However they still end up being the better choices as most other perks just still aren't as good in comparison.
I like Thrilling, Surge, Pop and Corrupt.
You can swap one of the slow downs out for I'm All Ears if you want. He's one of the killers that takes the most advantage of it and I still find it quite a mediocre perk.

This is what I use with him but it is not the strongest......I think....
M&A, BBQ, corrupt, and Infectious.
But I sometimes change M&A for stbfl.
Maybe you will play him differently but if Infectious pops after downing someone, cage and go for the other.
If they are not tormented then slug for a bit, hit the other survivor, pick the slug and hook.

Thank you both! This helps a great deal!

Copy-pasted from another thread but haha here we go.
Perks:
I'm All Ears
Infectious Fright
BBQ
Pop Goes The Weasel
Add-ons:
Valteil-Sect Photograph, Tablet of the Oppressor
Explanation
I'm All Ears provides invaluable wallhacks, which should never be underestimated on a killer who can hit through walls. BBQ is still one of the best perks in the game plus double bloodpoints, and Pop is the typical gen regression perk that's almost a requirement right now. Infectious is very helpful in knowing if you should cage or not, and provides some very solid information for decision making, which as PH can make or break the game. The add-ons increase the time you can use your power, and increase the recharge time of said power. Alternate add-ons you can use are Spearhead and Forgotten Videotape, basically the weaker versions of those add-ons.
If you wanna have some fun, go for double range add-ons (Burning Man Paining and Wax Doll), just be warned that using your ranged attack too frequently and missing can and will throw the game.
Also, as another tip: Only break god pallets. Your best bet of hitting Punishment of the Damned is when someone is stuck in an animation, vaulting windows/pallets or dropping them. Force people into a damned if you do damned if you don't scenario, since you can cancel your power almost instantly and get the hit, or get an easy m2. You can even deal with shack pallet being dropped like this, but it's still reccommended you break it.
This has been your friendly neighbourhood Pyramid Head Main, have a great day.

My go-to has been Corrupt / Pop / Tinkerer / Monitor.
PP head is really scary if he can get up close, and this build has most of the tools to get you there. If you are on top of the tinkerer procs, you get plenty of gen control through pop.
Monitor and Tinkerer keep survivors on their toes. I honestly think PP head jumpscares are spookier than any stealth killer because he has a huge character model, nobody expects tinkerer, and the Phead chase music abruptly starting out of nowhere is kind of a shock. I get more gen grabs with tinkerer Phead than ghostface just from sheer surprise factor.

Sloppy, Nurses, Ruin and either Deathbound or M&A.
He is a killer that, unlike hit-and-run stealth killers, you cannot be injured around. Making it as difficult as possible to heal can absolutely destroy even a coordinated team, and Ruin is just a good perk for him when it actually stays up for a bit.

Hey no worries!
I'm considering experimenting with Ruin/Undying (especially because I find myself getting less and less value from Pop and more and more from Ruin the more I play him) but for a consistent build I can't recommend ruin until we get something for totems (like Undying). I'll get back to you after like a week if I remember to, and I'll tell you if I think it's worth and what perk I'd take out for Undying.

Nurse's is almost mandatory.. being able to get hits through walls is something no other killer can do
I'm All Ears - same reason for Nurse's, helps end jungle gyms quicker if you can tell which way they're running while they can't see you
Pop because it's pop
Tinkerer or Trail of Torment.. stealth is good with him because, again, he can attack through walls

Amazing from what i can tell stealth works well with this killer

- STBFL (Even though it doesn't build tokens when using his ranged attack Its a good perk for all killers)
- Nurses Calling (because using POTD on a mending blendette is priceless)
- Discordance (because Surveillance is getting nerfed)
- Whispers (another fantastic perk any killer who wants to locate Survivors and not waste time in sections that have no Survivors in them. As PH has poor map pressure it works well with him - especially the last Survivor once hatch is closed just stay near the exit gate you hear Whispers at and as long as they don't have a key its a 4k)

Ahh great! Thats very helpful!

I play PH a lot(not claiming I am pro or anything like that), the most fun and possibly legit build I do with him is mostly knowing where the survivor is. In a way you know where people are and if your decent you can land those POTD (Punishment Of The Damned).
- Nurse's Calling
- I'm All Ears
- Barbecue & Chili
- Infectious Fright (You can also switch for Rancor or Iron Maiden)
This build is more fun rather than competitive so its really all up to the player. Good luck with your matches and see you in the fog.

Been playing him a lot in red ranks trying to find some decent builds, but it's entirely dependent on your playstyle of Triangle Man.
Even though his power doesn't have too much synergy with the pop/bbq combo, I still use it because the combo is just too powerful, and you're still gonna be hooking quite a lot of people in the trial.
Complete build is Pop,Bbq, Corrupt, and I'm All Ears (required if you ask me).
I only cage people under these circumstances: Decisive Strike. If I have a person I can immediately start chasing. If I am aware of the other survivors location. If I don't need Pop or Bbq. If I want to create artificial map pressure by splitting the team. Overall I try to cage on second hook.
You will still get a lot of use out of Pop and Bbq and can even use Bbq to see caged survivors if you're quick at downing, which you should be as PH.

Not a build, as I'm sure you'll get plenty of those, but a Perk that is surprisingly effective on Pyramid Head:
Franklin's Demise
Knocking Items out of survivors hands has its uses, but on certain killers, you gain more advantages. Hag and Trappers are obvious: Traps need Bait, but For PH, it's all about getting more usage from his power than is normally possible.
Lets face it, PH's Rites of Judgement Trails are garbage. Not only are they easy to see and hear, but survivors can completely ignore getting tormented just by crouching over them. I've had a plethora of matches where, no matter where I put those damn trails, Nobody gets tormented; Hell, Survivors will even crouch-walk over a trail, MID-FREAKING-CHASE, to avoid getting tormented! Luckily this is where FD shines best. When survivors retrieve items off of the ground, they are forced to stand up during the pick up animation, and coincidentally this actually counts against them when it come to Rites of Judgement trails.
You want survivors to get tormented consistently? Bring in FD, Knock items out of their hands and Draw a Trail over them. If you also use the Mannequin's Foot add-on, that extends the time your trails last in the environment to 95 seconds, and with FD making items only last 90 before they're destroyed... Survivors have no choice but to get tormented if they don't want to lose their item.
Even More fun with PH and his hatred of items: If you knock down a tormented survivor with his Punishment of the Damned attack, send them to a cage, and they Die in that cage with their item in hand... No other survivor will ever retrieve that item. When survivors die on the hook, the item they were carrying falls at their feet; same goes for the cages, except the cages themselves sit on top of the items and block anyone from reaching them.
All in all, FD is just a wonderful perk on PH (you have no idea how many keys I've destroyed using FD on him), so if you're having problems getting survivors tormented, or issues with survivors bringing in too many baneful items, you absolutely can't go wrong with Franklin's Demise and this strategy of how to use it.
Just in-case anyone had any doubts about whether or not FD works as I've explained on PH:
Note: My own Audio wasn't recorded for some reason I can't at the moment figure out why, but I instructed my Friend to crouch walk to the item, and pick it up while holding crouch in order to prove FD's usefulness on PH. What you hear from him is responses to our conversation when testing the effectiveness of FD on Pyramid Head, and his insight on the Rites of Judgement Trails. His last comment is about how he never sees PH players using FD.

I never thought of that before! Thanks ill put this to use

Perfect! Ill make sure too put this info to good use!

I'm not a fan of Pop or BBQ on him because of the conflict with Cages. Every time you hook a Tormented survivor you effectively lose 10 seconds of time you could have saved by caging them. (For comparison you can cross a map end to end in 20 seconds, so 10 seconds is enough time to go half way across the map and start a new chase.) And every time you cage someone it's a lost chance to trigger Pop or BBQ.
I know those perks are "meta", but I honestly think Pyramid is better off using non-hook based perks. Ruin + Surveillance is good for instance, it gives great tracking info automatically and has gen slow down while Ruin is up. And even if Ruin is destroyed Surveillance is still ok on its own since you can still kick a gen to keep track of it. You can pair that with two other perks of your choice. Nurse's Calling is good on him for obvious reasons, so you can also try pairing that with Sloppy Butcher to make the heals take longer. Or you can try Monitor & Abuse if you want to get a little closer to people before chases start. (Trail of Torment is also a good one, although it has a bit of anti-synergy with Ruin since you can't kick a regressing gen.) Or if you're looking for additional long range tracking Discordance or Whispers are both great in different phases of the game and don't require hooks.

Main thing to remember is that his trail is area control. That also means you can use it to negate loops almost instantly if a Survivor leads you to one. If the Survivor still chooses to use it, you got yourself a tormented.

From the many games that I've played, I feel PGTW, Corrupt Intervention, Tinkerer, and BBQ are the best. Yes, I know there are 2 perks that require hooks and feels counter-intuitive to PH's power. But there were many occasions where Pop and BBQ helped slow down the game tremendously. After you hook a Survivor for the first time, you can use BBQ to track the next survivor or go to a gen to Pop it. When you down a survivor for the second time, that's when cage of atonement comes in handy. Tinkerer helps lead you to a gen that you need to pressure and also lets you use Punishment to attack the Survivors unexpectedly giving you that early edge in a chase. Overall I'd say it's really good.
