Use the report ticket system if you didn't do it using the in game system, posting it here will just get it removed.

Right now the report system is being worked on for consoles, as of yet there is no ETA on when this will be ready so please DO NOT open a Support ticket for this - this is only for additional information to go along with an in-game report.
