Count the Survivor Going to Stage-2 on the Hook via Escape Attempts as Another Hook
The title isn't as clear as I'd like it to be. There is a rash of people either suiciding on the hook, or simply attempting to get off the hook themselves rather than waiting for their teammates (usually the safer more sensible approach). Thus, we have a lot more people dying on their first hook, or on their 2nd at most.
I would suggest that if the Survivors on the hook make attempts to get off the hook themselves that it grant the Killer another Hook if/when they go to Stage-2. *If they do not attempt to get off the hook themselves, and simply go to that stage at the end of the normal time increment, it would NOT provide the bonus hook.
I make this suggestion because fewer Survivors are going to suicide on the hook or engage in the risky behavior for no good reason if they KNOW they are actually rewarding the Killer by doing so. This would serve as a soft deterrent to a self-destructive behavior. It likewise might make sure some of those Survivors live long enough to help the rest of their team.

Personally the only time i would like to see this is they actually get themselves off hook. I've had times where i was about to go into struggle so i self unhooked only to then die on 2nd hook. Otherwise trying to escape the hook yourself deserves full punishment.

This will encourage more slug builds especially on high mobility killers. They'll down/hook everyone at once and then be rewarded with unearned extra hook points if anyone tries and fails to unhook themselves. I'd rather my teammates attempt escape as I'm running towards them or totally hook suicide than have more killers running slug builds.

Potentially. I'm not sure it would or wouldn't because it would be easy enough to set it so once all four Survivors are down, this effect doesn't trigger. And to be honest, it can't encourage MORE slug builds than we already have. :)

I do not understand very well. Another stage as if the survivor had never gone to P2, if this survivor tried (and failed) his attempts?
If that's it, it's like suggesting that the attempts do not apply any penalties (from the survivor's perspective) on the progress of the stage. It's free.
Yes why not. It's not a huge buff because of the odds, and it can have some interesting consequences. Suicide is no longer possible, indirect buff to Slippery Meat. This can encourage other survivors not to necessarily come directly to help (since they may come for nothing).

No, what I'm suggesting is that IF the hooked Survivor makes attempts to unhook themselves, and end up going to Stage-2 on that Hook, the Killer gets credit for another Hook. If they don't try to do that and/or are rescued before they go to Stage-2, the Killer doesn't get this bonus hook credit. The idea is to discourage, softly, committing suicide on the hook.
