I've had that happen to me on a hill in the swamp maps.
Couldn't pick up the survivor.

Shaq is broken atm. Sometimes when you're down and is sticked to the wall you also can't get picked up. Sometimes when you loop there's invisible colligions stopping you. It feels rare when it happens but when it does, feels bad.

Its a weird bug, been around for the last few patches if not longer that I am aware of but basically...
If you knock down a survivor and there head falls into a collision object like a generator, wall, etc... You can't pick them up unless they crawl out of that space.
