Escaping with new item but not receiving it

I was playing on ormond. At the beginning of the game I had a brown toolobox. Remembered my dead teammate had a flashlight. I went to the broken hook, saw there was a purple flashlight under it, so I picked it up. Then, I escaped and it wasnt in my inventory :(
Platform is steam.
It wasn't a white ward, or it wouldn't have let me pick it up.
This has definitely happened to me before, I was so confused.

I noticed the same thing. But mine was with an item I picked up in a chest. :(
