Suffer a 5% hindered penalty while exhausted.

I think it'd be too strong, especially when combined with perks like fixated or vigil. Maybe if this was applied to no mither or something else than it could be balanced while also making a worthless perk viable.

Probably too weak.
You would still need 80 seconds to recover Exhaustion and at this point, the Killer messes up massively if they need 80 seconds to get a Down against a Survivor who is moving at 95% Speed.

I have a hard time seeing how a survivor could last 80 seconds long in a chase. Without the hindered effect, it might be solid. But it could also be a dangerous perk. I don't know, the thing with this idea is is that it will help you extend chases massively only if you are already doing well and running the killer for at least 80 seconds. Meanwhile if you are already struggling to last longh enough in a chase, the perk won't do anything for you.
So it's kind of just a perk that is beating down on killers that are already struggling. Although it probably wouldn't be the only perk.
But if a perk like this ever were to happen, please without the hindered effect. If anything, a different, less crippling downside. Reduced action speed for example.

It's bad, because you will not be able to get any long chases because of hindered penalty unless you drop every pallet on map, but it would be super broken without it, so I would say impossible to balance.
