Top 5 Most Broken DbD Mechanics
I wanted make list for broken stuffs on DbD and sadly they are here for long time. This list is just my opinion, so they can be not true for you. We can discuss about it but please be respectful! Lets start.
Number 5: Mother-Daughter Ring = Spirit was one of the most hated killers because survivors were clueless against her. Anyway this fixed, Spirit is fine now. But this add-on is still so strong and BHVR did not touch it, yet. It makes Spirit really so fast and you have not so much against this. If you are near to good pallet or windows, maybe this can save you.
Number 4: Alchemist's Ring = This add-on is stronger than Blight's Irri add-ons and this tells so much about Alch. Blight is fine killer but this add-on is so busted. Most Blights are using this add-on because it is resetting your rushs for rush hit. This add-on only issue for Blight and needing change.
Number 3: Iron Will = This perk was fine actually but BHVR nerfed Stridor and now IW is problem. Because there is not any counterplay for this perk. You can play around of other survivor perks but for IW, you can't. This perk is so busted and so strong. Stridor should buff to pre-nerf and then IW is okey again.
Number 2: CoH = This is new meta perk for survivors after years. Most survivors swapped UB with this perk. Heal speed was not problem, infinite part is issue. Bless totem near to finished generator. Killer will never patrol this area. Now survivors have great area for heal. This perk killed all hit and leave killers. This perk already nerfed twice but still problem.
Number 1: Dead Hard = DH is probably most broken perk in game. It is 3rd heal state for survivors. And this perk makes maps more unbalanced. How? Most maps have so much pallets and vaults and this perk makes survivors play greedy. Probably pallets would drop early. But survivors are trusting DH and they are playing more greedy and this makes chases longer. This perk does not need nerf, it needs rework.
Bonus Number: Nurse = Probably i would put this to Number 6 but this is Top 5 list. Nurse is also broken. She is ignoring everything survivors have. And except her, no one can do this. Great anti-loop and great map mobility. She is another bad design and need changes.

Regarding numbers 5 and 4: god forbid killers occasionally have some good stuff amongst all the bad add-ons no one wants to use. Especially your complaint about MDR "you have not so much against this", what says that you should have other than survivor entitlement? It's an iridescent add-on, it should be strong. You not having much to counter it, is a good thing, otherwise that's a waste of bloodpoints.

Personally I don't mind mother-daughter ring unless it's paired with the cherry blossom add-on, which literally shows the spirit where a survivor is while she's phasing. I'd say cherry blossom is more op than mother-daughter ring tbh but I don't really have a problem with spirit anymore since hardly anyone plays her since her nerf, so going against these add-ons doesn't really bother me since they're so rare.

Chasing nurse is pretty viable unless nurse has wall-hack, calling her "bad design" and "broken" is definitely not right.
I don't know how alchemist ring can be strong but beside that I think I agree with it.

Boil Over on certain maps (just had a match with RPD offering wiith 2 boil overs. I afked in the basement. Not giving people bloodpoints)
Decisive strike should de activate when gens are done in my opinion.

CoH. Nerd did nothing. Still super broken. Shadow step is broken on inside maps.

Lets take key & hatch, Anti-Haemorrhagic Syringe nerfs back then? They are pink and they should be strong. Otherwise that's a waste of bloodpoints.
I disagree with you. Yeah, Irri add-ons should be strong but they should not be on broken statu. MDR is not gives any chance to survivor, it is lose or lose game.
Alchemist is purple btw.

Survivor perks, items and pretty much anything is generally lose or lose for killers though.
And yeah, I'm definitely on it as long as I can have old mori back!

At least the DS change is your opinion because dbd would die in one day with that change. Everyone will get tunneled out.
*EDIT* I didn't read the 'DE' activate. But again why that change, DS is most for the endgame.

This add-ons are also problem but at least Myers needs stalk you so long. And it gives chance to enough time finish generators. And plus, you can use all pallets against him and if he is close, jump to locker.
But Alch Ring? I don't wanna talk about it. It is super popular and that's tell everything.

I just personally aint a fan of DS. Many survivors abuse it and i think when the gens are done and doors are activated there should not be skills that are abusable as much as DS is. Thats just my personal opinion. Me i just eat the DS early game so i can get them out of the game.
That being said in my list DS is the least of my concerns right now. I just had 2 groups back to back abusing flip flip and boil over and i had to afk one of them in the basement and 1 i had to bleed out the last 2 survivors..thats not great gameplay. Also the CoH..i freaking hate the sound of boons activating.

Iron Will is very strong, being able to hear survivors is critical as a killer. I lose track of more survivors due to Iron Will than probably any other perk.
I don’t necessarily think Iron Will is “game breaking”, but I do think it should probably be a 90% reduction instead of 100% so that Stridor can at least still in principle amplify sounds of Iron Will survivors. They’d still be very hard to hear at 90%, even with Stridor equipped, but at least Stridor would have an effect.

You can have old hatch back, sure thing, because it did have a counter, which still exists. You slug the second-to-last survivor and go hunt the last one down. Slugging for 4k is still a thing and it works. It doesn't help you to know where the hatch is if you're still not gonna get it.
The syringe may not have had a counter, but it wasn't a game changer, it usually just extended a chase, which is part of the game. Whether you heal instantly or in 16 seconds shouldn't matter to a good looper, the resulting effect is then the same.
As with keys, you're just shooting yourself in the foot here. Keys actually had zero counter. If Survivors had one and found the hatch, it was game-over, there was literally nothing you could do to prevent their escape at that point. You can play around MDR, you can play around Alchemist Ring, but you couldn't play around keys, at least not the ones found mid-trial in a chest. As far as I am concerned, and you should be too, if you were honest with yourself, those examples are not comparable.
And so what about Alchemist Ring being very rare? It's the fourth out of five tiers of add-ons, those should be strong too.
My opinion is not that ultra rares or very rares should be strong to the point of being either uncounterable or broken, otherwise they are wasted bloodpoints, so don't imply that that is my belief. Neither MDR nor AC are uncounterable nor broken, which is why I think they are fine to exist in DbD and so would have the old hatch, for example.

The thing with Iron Will, it's either 100% or it's useless, anyone with moderately ok headphones will hear a survivor crying at 10% it's honestly not that difficult.

I agree completely with your list. key/hatch was a busted mechanic and needed fixed. Syringe is still very strong 16 secs in chase is cake these days. MDR/Tombstone, Alc ring are how purple/red add-ons should be....not make footsteps louder in dream world. I agree with your list but at the same time add-ons those said add-ons are how killers should be. Freddy and PH is a clear example of how not to be.

Hard disagree on Nurse. She follows Nurse rules, not conventional killer rules. Nurse chase mechanics are an entirely separate thing. A pallet is played in a different manner (yes, you can use them). A jungle gym is used differently. The junk tiles at the walls of the map become viable structures. Most survivors don't understand that and stick to what they know, which gets them dropped in under 20 seconds.
Everything else I agree with for the most part. Especially Iron Will. That perk really needs a change.

Mother-Daughter Ring/Alchemist Ring should be changed.
I think IW isn't overpowered, but should be changed because it crowds out other similar perks. There's no reason to use other similar "hide" perks when IW does it better most of the time for no condition other than just being injured. Changing it to 75% or whatever would make other such perks more valuable for the 100%.
CoH is not the problem. The problem is uncapped healing speed bonuses and how strong reset perks are against weaker M1s that rely on a hit-and-run tactic that any good reset perk kills.
Devs have already explicitly acknowledged the correct design issue with Dead Hard (the distance) as well as working on improving the clarity of feedback such as validation/working to fix weird interactions with specific powers on the i-frame. They specifically stated they don't want Dead Hard to be usable for unconditional distance the way it is now and that it makes it too strong.
Nurse is the best balanced and designed killer in the game. Her chases are literally the epitome of "better player succeeds". Make or fake the mindgame read better.

Footstep noises are useful but some maps they’re soft or non-existent, and they don’t help when the survivor is crouching.

I do tracking perks a lot too. Even so, when I do lose someone it’s typically because they are crouching and I can’t hear them, not many perks deal with that other than Stridor or Bloodhound or sometimes an aura perk if it just happens to trigger at that moment.

It is because it has not any counter. It makes survivor 100% silent. Stridor countered it before nerf but not anymore. I just would take back Stridor nerf.

If you would just revert Stridor, you will still only have 1 Killer using Stridor - Spirit.
Before the Stridor-Nerf, you only saw it on Spirit. Every other Killer player who had a problem with Iron Will did not bother to run Stridor anyway.
