Assuming shenanigans, I've dodged.

That's Steam Achievement Manager, most likely.
You can report it if you want, but considering it has been around since 2008 and I have never heard of a single person getting banned for it... yeah, unless they combine it with any in-game cheating I doubt anything will happen.

I guess you could report them on Steam instead of reporting to BHVR?
Not exactly sure how the reporting system there works as I at most have reported specific comments or user name and nothing else

That was an achievement unlocker, some people use them and while they are not a guarantee of the person using cheats is usually suspicious enough to dodge.
You can report it to Steam, if they used an achievement unlocker most achievements should be unlocked the same day at the same hour, they may not do anything but reporting on steam takes like 30 seconds so better safe than sorry.
