I guess most of them come with the clothes they have been taken with by the Entity. Different clothes for me indicated they are from a different universe.
though this is not the case for every cosmetic - eg the Legion bunny costumes have a description in-game that imply they found them in the fog iirc. And the blighted cosmetics are also very likely obtained in the fog.

I like to think there’s this whole world within the entity’s realm. For survivors and killers who please them, the entity gives more luxuries and amenities, to the point where their life resembles some form of normalcy while not within a trial. Its mostly an illusion of course but real enough for most to believe it.
Of course the Entity erases memories selectively which encourages certain behaviour. I sometimes think about writing a story about how all this works. The Entity strikes me as someone as timeless and thus extremely patient and knows you can get more bees with honey.
For those who underperform they take things away and give them less. For those who outright defy them such as refusing to hook or refusing to attempt to survive because they are so devoid of hope? Pain. If that doesn’t work, the void. There’s a reason why none of the survivors date back any further then Bill, yet you have killers from ancient Babylon and the 1700s-1800s.

Since the world of the Entity is basically made of memories via auric cells, The Entity potentially just sometimes summons them back to the campfire with clothes it fetched from their memories.
It's also stated in lore that there are 'multiverses' that feature our characters existing via different routes/versions of Earth, or other planets etc. for example Haddie in the tome stories is different to the one we got in game, and the Observer has seen Dwight exist in a world where he was an Emperor, so it's possible that it reaches into these memories/versions of them and basically makes their outfits out of those potential realities.
There are also situations where what happens within the realms also provides them with their cosmetics, i.e. Blighted.

in my headcanon the outfits are all from other variants of the survivors from other dimensions/universes, since it is confirmed through in game lore that there are multiple universes and versions of the characters, and also by judging how some item descriptions feel like they would not match the personality/lore of the character as we know them. I guess they just kind of wake up after dying/escaping a trial and find themselves wearing something randomly chosen by The Entity and just go along with it.

Lol, imagine waking up in such a nightmare and additionally to all the ######### you have to endure during a trial you have to carry around a backpack and paddles or ski blades or whatever on your back that you can’t lay off

The entity takes survivors and killers from different realities. That's why dwight has so many different job outfits and yet he's so young. Office worker, pizza boy, camp counselor and so forth.
The entity takes clothes and merges them with every survior already within the realm. That's why newer ones get many clothes fast, the entity wants to play dress up.

They probably scavenge them from the buildings in between trials.
I mean, so many closets and stuff around. Why not take something with you? XD.
Maybe they do it during the trial. If they can take a medkit with them why not a new pair of pants?

I feel like the 'Fog' doesn't necessarily refer to the realm exclusively. I feel like it could also relate to 'brain fog' as in the Killers and Survivors are both in a kind of haze. The survivors are probably summoned in a random outfit in each trial, and at a glance they recognize their outfit and don't think twice about it. More specifically they probably don't think about how they got there or what they were doing before entering.

What if The Entity gives Trickster a vanity with makeup, colongue, and nice amenities so that Ji-Woon can keep up his appearance.

With the exception of certain skins like the big mascot outfits for Legion/Blighted outfits/etc that say otherwise, all of them are canonically other timelines. For example, the Lunar New Year Rat Susie skin is from a timeline where the Entity snatched her from a Lunar New Year's festival instead of the "original" Legion's origin-point. It's not the characters "get" their clothes. Trickster's Brazil skin is from a timeline he got kidnapped from Brazil instead. So on and so forth.
