how is it possible to still hatch camp?
Just had a match where survivor 3 was on the hook, and I went hunting for survivor 4 and found her over by one of the combines on farm, and she was just standing there. I downed her, and when survivor 3 finished sacrificing the hatch spawned 2 meters away from where she'd been standing. (I picked her up and dunked her in the hatch, because she'd played well and I wanted to reward it.)
I guess it could be a HUGE coincidence, but it seemed like she was camping it. Nobody used hatch offerings, and she didn't have the hatch-finding perk (which wouldn't work anyway until the hatch spawned). She's also a "globe" player, which means she's probably playing on console, so even less likely to be a hack (although she could be playing on non-Steam PC, which I guess would be slightly more likely to hack?) but she didn't do anything else during the match that looked hacky.
Any ideas? Very subtle hack? Just a big coincidence?

Most likely a luck but hacker exists.

This^. The combine is also a common hiding spot for survivors. Just a coincidence really.
Unless, let's say she had a green map with silk cord and no range addons nor twine and she noticed map scout events at the combine. Then, deduced hatch was at that spawning point based on number of nearby gens and score events.
Very long shot, but possible.

The hatch is actually a non-interactable object the whole game. While the hatch is invisible, your character will still rise a lil, but noticable, bit when running over the hatch; that survivor might have just discovered the hatch earlier that game and thus returned, when the situation turned dire.

I've had the Hatch spawn right under my feet plenty of times

I'm not 100% sure, but I'm pretty sure that they reworked this mechanic back when they changed the hatch. When they first had that update that changed hatch to spawn with only 1 survivor left, a friend and I took a hatch offering for shack and walked all around and in shack and we didn't raise at all.
I will also say that before they had the update, I would find invisible hatch every 1/5 games maybe just from being in chase or running around for a gen. I haven't witnessed invisible hatch since then... so pretty sure this is outdated information.

I feel like people are a little over-paranoid about hacks. Even before the hatch change, there would be times when the hatch spawned under my feet. While it is mostly luck, after you play a certain number of matches on a map, you get a feel for where the hatch does and doesn't spawn, so you know where to start looking to better your odds. I highly doubt there was any funny business going on.

I also heard that, if you're standing where the hatch will spawn, you can't do things like heal yourself
So if you're REALLY observant, it is possible

Dude, I'm too busy running to notice that at all. XD

Yep, besides someone hacking to get hatch makes no sense, cheaters tend to get more juice out of their cheats, even the subtle ones you notice there is something off because they either get too many extra loops or you have to throw the pallet much sooner than you tought.
