Do you have to actually use self care to heal yourself, or can you just use a medkit while having the perk in your loadout?

In any challenge that has a perk in it it just has to be equipped

Just like giving Nurse AND Blight prominent features in this tome, including a sweaty af "kill all 4 survivors,let no one escape" master challenge.
