Head On Randomly Deactivates?

As you can see from the provided link, I wasn't currently exhausted and the killer was in very close range of the locker. I also noticed that the survivor didn't make any noise when I fast-vaulted. I'm not sure if this will help replicate the bug but a crow had appeared above the locker beforehand.
