Decisive strike bugged

Platform: PC through Steam
Issue: when using decisive strike in multiple situations either the game glitches and my model is flying around or the killer doesn't get stunned
Frequency: it's happened 2/3 times in 3 games I attempted to use Decisive

This exact thing (flying around while still in the carry animation - see first video) has happened to me 3 out of 4 times when succeeding the decisive strike skill check in the past week.
