Does a Lullaby count as a Terror Radius for Perks?
I am a Freddy main (for killer) and I'm looking at trying to complete more challenges in the Tomes.
If the Survivors are asleep, can hear my lullaby, will they receive the Exposed status from the perk Starstruck if they are within the range when I pick up a dying Survivor?
Does Distressing increase the distance of my lullaby as well if they are asleep?

The Terror Radius is the "heartbeat", so Distressing will expand the heartbeat that Survivors hear. Lullabys (like Freddy, Huntress, etc) are not counted as a Terror Radius, so (for an example) perks like Insidious will not supress a lullaby like it does the heartbeat.

What about HEX: Plaything? Will Survivors affected by that hear my lullaby?

I could be wrong but if a survivor is in the dream world they’re already oblivious and can ONLY hear your lullaby in place of a TR. So Plaything would only be effective if they’re not asleep?

if the Survivors are awake they'll get exposed but if they're asleep they won't get affected by starstruck cuz They hear no terror radius.
no perks affect lullabies so using tr related perks on freddy is usually a bad idea.

No. No matter which perk is it. No perks affect the lullaby, mostly due to the inconsistence that would come with it. So in example if you want to use a perk that has an efffect on supressing the TR, they will still hear the lullaby as lullaby is not the same as TR.
