Stuttery/Jumpy/Hitching/Frame Lag is randomly back now , please help me

For context. Over the years I have had this happen multiple times where DBD as survivor mainly and with camera movement will just jump around an freeze and hitch or lag and stutter, what ever you want to call it. This has happened across diferent entire PCS and all diferent peripherals, I have tried all types of different settings. I literally had this happening on my previous PC and got so fed up I built a brand new gaming PC only to have this happening right off the bat again. Weird thing is then after some time the game becomes playable for months and I dont have a major issue. Just recently I noticed a little bit of performance lag and decided to do a clean remove and install of NVIDIA drivers and it is WAY worse . I have literally scoured every posibility and cannot figure out what this is and how to remedy it . Its so ridiculously frustrating.

Its not internet lag, it happens on tutorial match and my internet is good.

Ive tried every combination of G Sync and V sync but I typically have g sync on and v sync off, FPS120 in game and AA and V syn off
