That is a Claw Trap. They get attached to you when you're scanned by a Drone 3 times. Once it's attached, it'll take a health state, stop you from healing and constantly show you on the Skull Merchant's radar.
Getting scanned by a Drone again while you have a Claw Trap attached will reveal you with Killer Instinct and slow you down.
When the battery dies (i think it's like 45 seconds by default), it'll just detach by itself. There's no way to take it off early though.

Yes, after some time. But those stacks you have you can't get rid of for some reason. When you're on 2 stacks you will get injured + deep wound + hindered at some point

That's actually not entirely true- You can use Plot Twist to remove them
