Doctor extra madness level

For some reason, there seems to be a fourth type of madness HUD showing up with the doctor.

It's pictured above being compared to Madness 3 (below). Functionally, it appears to work as Madness 2. The screenshots provided were taken shortly after each other. I have been unable to consistently replicate it.

For reference I'm on the Steam version of the game running: Lethal Pursuer, STBFL, Surge, and Pain Res with Yellow Order and Restraint add-ons (Class II)

    Pretty sure that's normal. It's been like that forever, at least to my memory
