PC - phantom skill checks while walking around

Started getting phantom skill checks while walking around in the Gideon map playing against the Dark Lord. No interaction with them, the skill check noise and the visual briefly appeared and disappeared repeatedly. It seemed to fix itself when I failed my first skill check after starting a generator.
Was playing Dwight, with the perks Deja Vu, Deception, Flashbang and Head On, and was affected by Kindred and Open-Handed at the time.
Unfortunately i did not think to grab any game logs at the time.
