Not a Bug · Last Updated

You were hit by the killers power attack not their basic attack - exposed status effect only gives the insta down if used together with a basic attack

    This is not a bug.

The Nurse blinked at least once before hitting you, and her blink attacks are considered Special Attacks. This was a nerf that was implemented a long time ago, and it seems the Nurse player didn't get the memo.

Sloppy Butcher, Surge, anything that applies exposed, are all examples of perks that require a Basic Attack to trigger, which the Nurse can still do, but only if she does not blink before hitting. Any attack after a blink is a Special Attack and will not trigger the effects of these perks.
