This also just happened to me with Cenobite as well, working to get adept and not getting it is fun.

This just happened to me on steam my user name is ECK2335. I got a 4K and actually met the old requirement for the adept on the Nightmare Freddy and did not get the achievement.\ My steam ID is : 76561198301492475
