Where does it say that's the nurses heart?. I'm also assuming the killers might cross paths but just like the survivors they too can get lost in the fog. Really the killers are prisoner just as much as the survivors

The Entities true form? I'm gonna say no. However the Entities Avatar, Absolutely. The Entities been very displeased with the killers lately and has decided to use a fraction of it's powers to make a Avatar to do the job it's self.
