Some of these low-poly issues have happened to me. It is especially noticeable during The Onryō’s condemned mori/kill screen (not very scary when the eye looks like a square lol). I can provide a screenshot if needed.

I have also had this same issue with stacks not transferring from third seal when undying is cleansed.
