This could be a bug, as there is a new rift outfit for the doctor with a weapon that looks exactly like the one in your second picture.

It says "rush can only be performed a maximum of # times", not "decreases rush tokens to #", so it might be that those just cap the tokens instead of actively decreasing them. This would mean that shredded notes decreases the token count from 5 to 4 but those addons cap it at 3 or 2. This is just a guess though and I could…
