I was playing the anniversary event, completed both generators in the basement on the new DnD map, and didn't get the achievement.
Tome 2 Level 1 has a challenge "Dead Hardy" where you have to "Use Dead Hard to avoid a basic attack by a nearby Killer 1 times".
