Rock spawning to extend loop in Family Residence
I don't know if it can happen in other maps, but I encountered this yesterday. Neither myself nor the killer could fit through the small gap between the rock and the wall, so the loop was extended.

Broken tiles in Azarov's Resting Place

Thrown inside of an object by Wesker's bound

Killer can completely block a hook in Hawkins
It's impossible to get the unhook if the killer decides to stand right where Wesker is standing in this screenshot. The anti-facecamping mechanic doesn't come into play either, since the killer is far enough away for the metre not to fill.
