This happens to me more often than not. It's to the point that I don't trust vaulting or dropping pallets unless I'm absolutely desperate because they feel like death traps.1 -

I honestly miss when hooks could be sabotaged to 99%, but Sabotaging has been nerfed, too. Now it's kinda useless.2 -
