I've about had it with this game and it's buggy mess with Trophies
I literally posted a couple days about wanting to attempt the Evil Incarnate Trophy before I Prestige my Michael Myers. My first ever time sitting down to attempt it. I mentally prepared, double checked on the information for it, picked out a Perk build imo, best as I could be suited for it. Even asked my GF to get into a KWF for some practice matches.....
The Trophy/Achievement is infamous for how easy to miss it is solely due to Survivors DCing, hiding in lockers, escaping via Hatch or dieing to Endgame Collapse...
Well my first try at it, people DC'd at the loading screen after seeing my Ward offering...second game for my first real attempt...I got all 4 Survivors killed via Tier 3....game didn't give me the Trophy....literally every Trophy I've earned, pops up on the endgame screen...so I knew something was wrong. I figured maybe after going thru some menus it'll maybe late pop? Nope...I sit..nothing...I play a normal match...nothing... So I decide to restart everything....welll this ######### game & its code had decided that I did NOTHING at all...I lost all the Bloodpoints I had gotten in,my 3 Matches and essentially reset my game's progress to when I just booted it up today.
I ######### literally had the Planets align to get me a good matchup where people either didn't know what I was doing or just weren't douchebags to deny my Trophy attempt.....and now all ######### morale and enjoyment today is gone, because this game can't handle doing Trophies or properly saving progress

Man, it's sad that my knee-jerk reaction is "If THIS low-priority thing is your tolerance limit, you're going to have a bad time with this game."
But I do not mean to belittle your frustration. This is a thing that should absolutely work right, especially on very difficult achievements like the one you're trying to finish.

I have dealt with plenty with this game, but yeah it's not high priority...but because unlike a lo of other Trophies/Achievements...this one kinda requires a lot to align to work and is easily thwarted. The fact I got it on my first try (yea yea low Rank) and was just denied it for no reason other than the game being broken just frustrates me. I wouldn't care if I messed up or had Survivors stop me...

I would agree that this would most likely pop up on the 'brutal killer' screen, and should have? It's possible the add-ons glitched it. Or it secretly wants you to get merciless killer along with it.
However, you should be getting trophies mid chase/in game.
Dmg 100 generators while someone is hooked. Sacrifice 25 ppl after gens powered. Open the basement chest 50 times, etc.
Sounds like you're losing hours of progress too. Might consider deleting & reinstalling.

I don't have any reports of this trophy being bugged on console as of right now I'm afraid. With regard to the loss of progress I can only direct you to support to submit a claim but I'm afraid they will be unable to give you the achievment...and I agree it's a really hard one to get!

I appreciate the response Mandy! This thread is a bit of s rant, but also some awareness. I don't known if the Progress bug/issue that's at the top of the page coincides with what happened to me or if it was something else.
I know it comes across as ridiculous or silly, but I have anxiety and self esteem issues and in addition to my job taking up pretty much a lot of my time (40+ hour weeks past 2 1/2 months) I jus have been frazzled and tired...to have had victory given to me on a first try made me the happiest person, to then have that deadening silence of no Trophy pop just kinds became a pallet slamming on,my head (ironically my job has me handle pallets a lot...lulz).
Hoping to try again today, even if a progress issue is about because I need to start Prestiging Michael before the coming weeks to take advantage of the upcoming events. Do or die, I guess?

I have a in going issue with PS4 trophies in general, did the 4 man hatch escape and got denied of the trophy, a couple of days ago I escaped using Bill (adept) and the game didn't register the achievement. On Dark Souls 3 I had experienced this as I had a Estus Flask +10 (the most you can get) and didn't get my trophy for upgrade the flask to maximum.
