How does map selection work?
Do we know how maps are selected? Do certain maps have more weight to them or do all maps have a certain % of being selected? Or are realms selected first before the map so technically something like Haddonfield would be more common than one of the Badham Preschool variants?

I believe it's completely random - that is, every map has an equal chance of being selected. If someone burns an offering to go to specific realm, the map is chosen randomly from all the maps in that realm.

Yes, it has already been stated that it's complete random.
Which it is, but some maps are chosen more often than others due to how many different versions of that map there is. So, for example we have about 5 Badham Pre-school maps, 5 Azarov and Macmillian maps, 2 Yamaoka maps soon, 2 Red Forest maps, etc.
You get the idea, that's kind of why you'll see a lot of Cold-wind farm maps, Azarov maps, Macmillian maps, Badham maps, etc... before you see something like Hawkins or Lerys.

I doubt The Entity favors the Killer over the Survivors, The Entity just enjoys the cat and mouse game they both can provide.
Also, I'm pretty sure the Killer has the same odds with an offering than a Survivor does with their offering. Say, cold-wind map offering vs The Game map offering. It's a 50/50.

Just like Fibijean stated. All is picked at random. (RNG) Until someone plays an offering.
If only a survivor plays a map offering then the survivor's map would be 100% picked.
if a survivor and the killer pick a map offering 75% of the time it will go to the Killer's picked map and 25% of the time to the survivor's
if all survivors pick a random map offering and the killer does as well the game will randomly choose one of the offerings and then go to that specific map.

Isn't it unfair for the Killer to have more favorable odds when putting their offering against a survivor? Or does the 4v1 aspect counteract this?

The entity favors the killer more than the survivor. So lore wise.. it will always be more favorable for the killer.
Game mechanic wise it makes sense if it's a 4v1 that the killer is more favorable due to some maps being highly survivor favored lol

Killer offered Hawkins offering, Survivors offered a disconnected while loading offering.
Comment afterwards was "you make an offering like that and don't expect one of us to disconnect?"
I muttered about Gideon being worse than Hawkins, but I remember what happened with the DLC Stranger Things and we had Hawkins all the time for like a week solid.
I assume that's the plan come December 3 when Oni Wan Kenobi and his cute Padawan appear? That we will get Yamoaka Estate version 2 a lot, like all the time? For a week?

I don't think that's how it works, unless Survivors/Killers intentionally put offerings to get the map, than it's random whether you get the map or not.
Like for example, when the Stranger Things DLC dropped, I got the Hawkins map like once for the entire first day on random while also getting it twice with offerings... than I got it 2 randomly and 2 times with offerings.
I was lucky to get every achievement on the first say except "Denied!" Because that achievement requires more than 1 day to do... anyways, it's 100% chance that you get the map randomly, while since it's a new DLC map, you'll see a decent amount of people use offerings a lot or not at all depending on the map itself being survivor sided or not.
Unless a Killer puts a offering up for a Killer dominant map, than you won't see that map a lot.
