Any idea what causes this bug? I've had it happen to me so many times and it's really frustrating as you can't see who is hurt, skill checks, any information on other survivors, and sometimes my exhaustion or wiggling won't work.

It would be a good idea to take some screenshots the next time this happens to you and post it in the Bug Reports section of the forum (link below) with your platform, description of what's happening and when, and any additional information you can think of such as what role you're playing, your loadout, the map, etc. The more information you can give, the easier it is for the bug team to identify and fix the issue.
