Leveling up a perk
I leveled up the nurse to level 40 to unlock a nurses calling. However I did this using the least BP possible and did not rank up the perk so it's still level one. My question is, do I have to level up the perk with the nurse to make level 3 usable for other characters? Or can I level the nurses calling to three without leveling it up with the nurse?

All you need to do is to just unlock the perk. You do not have to level it up to make it upgradeable on other characters.

Are you 100% sure? Or have a character similar? I know they did alot of patches with perk sharing and that's why I'm worried/asking

As sure you know, at levels: 30, 35 and 40 appear on the bloodweb the perks with orange color.
These are the ones you need buy to it can appear on other killers bloodweb.

Inferjus is right. I am 100% sure that you don't have to level the perk on Nurse to get it to tier 3 on other characters, as long as you have unlocked the teachable version at level 40.
For the sake of your peace of mind, though, I've taken some screenshots to prove it using Flip-Flop on my survivors. As you can see from the screenshots below, I have Ash at level 41, so all his teachables have been unlocked, but Ash himself only has Flip-Flop at tier 1. Feng, however, has Flip-Flop at tier 3. The same will be true for all characters and their perks, Nurse included. Hopefully that answers your question 😊

I mean yeah, once the perk is unlocked it's free-gain for you to level up on any Survivor/Killer, despite the original perk for THAT Killer/Survivor not being at Tier lll.
It's not a "that's so obvious" scenario of course, but it should be the case... and it is. I mean, it would be kind of odd if you couldn't say... level up Self-Care on Feng Min UNTIL Claudettes Self-Care is Tier lll, that would be kind of strange in my opinion.
