Is it safe to assume our cast of unfortunates have met this figure? By the way he talks in his journals and in the Archive, it seems that he exists outside of the trials but still in the Entity's realm, and it insinuates that the bonfire isn't the only safe haven outside of the fog. Have other survivors been where he has been?

I guess not. Nobody saw vigo. I heard about him. Maybe yes, but only because of his notes. He created the trap door, unfortunately, the kinematics show us that it was in a vacuum for an unknown period of time. There's made it out of there. It seems that you travel through the woods, in others, perhaps realms.
