i got hit through a wall and i also my perk head on didt work or get activated

so what happened was i was up against a wraith and i got hooked so that was fine untill i got hit through a wall and i said well thats strange and once again it was the end of the game when i tryed to use head on when the killer was 1 foot away from the locker but it didt work

Sounds like there was an amount of lag in the game, which meant that on the killers screen you weren't where you thought you were...if that makes sense. Which would mean, that although you saw the killer 1 foot from the locker in reality the killer was in a different place so out of the range of the Head On hitbox.
