does BHVR need to have licensing permission for levels or is it just characters because of the house from myers level comes from the movie

If the Map contains an iconic building of and is named after something of a trademarked brand (such as Haddonfield - Halloween), then they do need permission from the related company.

Anything that has to do with a license needs permission from the license holder, that's why The Pig and Michael Myers didn't have alternate skins for a long time, they needed permission to do it first.
