[PTB and PC LIVE server bug report] - sole survivor bug token

Hello,
I was doing a trial with "sole survivor" and during a game, some of my teamate die (or quit), after a death or sacrificed the perk trigger only one time and won't trigger when someone disconnect.
"Sole survivor" do not take more than 1 one token only by sacrifice and kill survivor disconnect doesn't not count and you can't reach more than 1 token
like right now one proof of this bug on the PTB
and this one from live.
