Unable to rescue from cage in elementary school

Killer was Pyramid Head, he caged Meg and it placed her in the upstairs corner of the map. I made my way up there and was unable to rescue her at any point in the process.
Not sure how to reproduce, I think it was a freak bug or had something to do with the fact the cage was facing a locker sideways.
Was the first time I've seen it, will edit if it happens multiple times.
