White ward not working on PS4

I just used a white ward, character died on hook, then when checking items: The used item was gone, as well as the two addons - but the white ward was still in the offering slot - AND NO WHITE WARD WAS USED. I still had the same number of white wards for that character as before the game started.
Platform: PS4
How often does this occur: Just did a minute ago.
