Weird Pyramid Head bug?

Does anyone else have the issue where you get randomly tormented. I don't have any videos but sometimes I'll be working in a gen or just walking around and out of the blue it acts like I walked over the little trail and gives me the torment debuff. Is this a common issue or just weird glitch?

    Yeah, it's been there since the PTB and the Devs didn't bother fixing it.

    I've been on a gen and I randomly get Tormented. Another reason I've been avoiding survivor .
