Survivor Physically Unable to be Saved from Hook (PC)

This may be specific to Grave of Glenvale since that is where it occurred but I'm not 100% so I'm not confident in putting it in map issues.

Killer: Huntress

Survivors: Meg Thomas (Hooked), Nea Karlsson, Adam Francis, Laurie Strode

A survivor was hooked on Grave of Glenvale on the hook located on one end of the main road and was being camped by the killer, therefore the remaining 3 survivors teamed up to attempt to save them. Upon running up to the survivor the prompt to save them on the bottom of the screen did not appear and attempting to hit M1 to pull them from the hook resulted in nothing happening. This occured when two separate people tried to save the same survivor during the same hook phase. This resulted in the survivors being forced to leave the hooked survivor to die and resulted in the killer being able to down another survivor and hook them for a second kill when without this bug all 4 survivors would have survived.

This only occured once and I'm unsure how to replicate it, or how often it may occur.
