This is on PC, this is the first I've seen it but I saw nothing there and somehow ran through that stuff to get Tormented?
The match had literally just started as you can tell from this clip... no heartbeat or anything so it wasn't a matter of "Was he below us and it teleported up top?" and even the killer who popped into my chat was like "Ok... ima just walk away now..." after seeing that I was somehow detectable for a few seconds at the very start.
